the new GED from conitech has one.
I think it is Beta now....
that is probably the best.

(alternatively)
you can also use MED to make terrain heighmaps.
Look into the terrain shader that comes with
gamestudio. i at one time used a paint program with it, and
got decent results.

if you do a google search, you can find others,
but to use multitexturing might require extra work....

I believe there is a plug-in for earth sculptor
that looked good,
but I don't know much about it.

I am sorry, I didn't read your post well.
You were looking only for Earth Sculptor...

Someone made an exporter for Earth sculptor recently.
I will try to do a search for it....
I think I remember trying Earth Sculptor directly,
I think it has an export to heightmap (MED imports heightmap),
and also would have an image that would go with it.

If it had an .obj exporter, you can import it into MED that way too.

edit: yes, I checked it, it exports to .obj
but using the exported heightmaps and
multitexturing could be a little tricky.

Earth sculptor can export .bmp heightmaps. These can be imported into MED:
File->Import->Terrain from image
