More camera angle problems

How do I keep the camera constantly behind the player a certain distance. It's easy to work with level coordinates, but in this case I wanna work with coordinates relative to the player. Thanks a ton!

Edit: I guess to extend on this, how can you position anything on an entities coordinate axis instead of the world axis.
