from manual 1 - total number of mesh vertices, including duplicated vertices with the same xyz position, but different skins or uv positions. So whence came this "duplicated vertices" and what they need? fantasyhammer.mdl (do not have second uv set)

Every face seems to be separated. Make a backup of the model, then select all vertices and hit the "merge" button in MED. I bet it will solve your problem.

I think in the wiki is a documentation of the .mdl format. So it should be possible to read this out from the .mdl file.
