i speak english natively and i still dont know. not much context there, but if i'd have to guess i would say you were right.

the sentence part provides the full context, something like "there is a physical examination and immunizations are the form.".

yeah, then your probably right, they are probably saying that there will be the physical, and formally immunizations are needed/given. still a very odd way to say that.
