When I export a model with custom smoothing groups from 3dsmax 2009 by FBX(v2010 or other versions), the smoothing group information has been lost in MED.

I also tried with 3dsmax 2013 but the result is same.

When I import this fbx to milkshape, smoothing groups is working correctly in milkshape. So there is smoothing group information data in FBX file. But MED can't import this data.

why you dont export as 3ds file from max?

Because 3ds fie format doesn't support animation, bone and second uv map.

3DS also does not support smoothing groups, either. But please contact support and send them that FBX model. They'll look into it.
