Forgot that R* was owned by Take-Two. I remember Take-Two mostly because of the Battlecruiser 3000 Debacle... fun to watch, horrible to live.

The Hot Coffee mode, AFAIK, was put in as a joke by some developers and just forgotten to take out... it was never meant to be released and I would chaulk it up to an internal quality control issue.
