if you use photoshop, make a new pic in any size press filter > render > clouds.
Save your pic, open MED choose file > import from pic, and then you have a smooth terrain. If you want to customize you terrain, the only thing you need to do. Is taking the brush tool, and add some black or white to the pic. White make the terrain higher and black make it lower.
