shoulder:

camera.x=player.x-100*cos(camera.pan+30);
camera.y=player.y-100*sin(camera.pan+30);
camera.z=player.z+30;

other cam:

camera.x=player.x-camera_distance*cos(camera.pan)*cos(camera.tilt);
camera.y=player.y-camera_distance*sin(camera.pan)*cos(camera.tilt);
camera.z=player.z-camera_distance*sin(camera.tilt);
