Whether I used v-sync as on or off it made no difference. The d3d_triplebuffer setting made absolutely no difference
Same here on my Nvidia card.
I agree Why_Do_I_Die that 64 fps is enough, but large changes in code seem to have no effect on the fps unless it lowers it below this number. This makes it difficult to optimize code for slower machines since I cant tell if the game has the potential to run above 64 fps with any certainty.
