Models render the fastest. You can have 100,000 visible polygons and still get past 60 fps on an old Radeon 9600 XT without updated drivers.

There's a whole host of choices. MED comes with Gamestudio but is very limited and quite buggy (mostly minor bugs though). Along with 3DS Max and Maya, there's Blender, Wings 3D, Terragen (used for making terrains, not general models), and likely many more.

I use MED to make levels with. Since model-only levels are much faster than levels made with blocks allowing for far more detail in shape and design (and near-instant build times), you should use some sort of modelling program for this.
