If you don't mind spending a few dollars (about $50; WELL worth it), you can pick up UltimateUnwrap3D. Here's a list of all the import/export it can do:

I import into it, do my UV mapping, then save as .md5. Open .md5 in MED, check to make sure the skins are ok, then re-save as .md7. Works like a charm.
